[job_card.job_description]Assure appropriate care of patient is provided through admission assessments, care planning, goal setting, evaluation, coordination of care, and emergency / safety plans. Complete OASIS assessments at prescribed time points including but not limited to Admission / Start of Care. Identify potential referrals for services from other disciplines that have not yet been ordered by the physician. Collaborate with clients, families, physicians and other members of the patient care team. Valid license as a Registered Nurse in the state of Minnesota Two years of home care experience including strong working knowledge of OASIS assessment Fluent in the English language Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies Valid driver's license and proof of adequate insurance
