Role Overview

As an RN Case Manager, you coordinate and deliver high‑quality, patient‑centered home health care. Your responsibilities include:

• Providing direct care to patients with acute and chronic conditions in their homes
• Documenting assessments and validating patient needs
• Promoting patient autonomy and helping them adapt to changing health situations
• Applying strong critical‑thinking skills to ensure safety and comfort
• Preventing adverse events and supporting positive health outcomes

Qualifications

• Current RN license
• Strong emotional intelligence, compassion, and empathy
• Ability to navigate challenges and resolve conflict

Schedule

Full‑time, Monday-Friday, 8:00 AM-5:00 PM.
