Candidate Qualifications:

• Board-certified or board-eligible in Family Medicine

Responsibilities & Duties:

• Deliver comprehensive primary care services to patients of all ages, to promote preventive health and wellness and treat conditions in a patient-centered setting.

• Collaborate with a multidisciplinary team to develop individualized care plans, coordinate referrals, and engage in community health initiatives to improve overall patient outcomes.
