Overview

We are seeking a Physical Therapist to join our team!

This position will provide physical therapy services in Austin, Blooming Prairie and New Richland, MN

Duties & Responsibilities :

• Provides direct and indirect physical therapy services in long term care facilities, home health, assisted living and out-patient facilities
• Maintains accurate record of therapy sessions, consultations and other pertinent activities
• Performs other job-related duties as assigned.

This position would require working with out-patients in Austin, MN, Blooming Prairie nursing home and New Richland nursing home, providing evaluations, treatments and supervised physical therapy visits with a physical therapist assistant for home health and three assisted living facilities in Austin, MN.

You will plan physical rehabilitative programs to improve patient quality of life.

Responsibilities :

Develop and implement physical therapy programs that focus on rehabilitation

Work directly with patients to achieve maximum physical recovery

Evaluate effects of therapy treatment and communicate patients' progress

Record and document patient care services

Collaborate with other team personnel to achieve well-rounded care

Qualifications :

Previous experience in physical therapy, rehabilitation, or other related field.

Ability to handle physical workload

Ability to build rapport with patients

Excellent written and verbal communication skills

Strong leadership qualities
