Asthe RegionalClinical MedicalDirectorof the comprehensive care delivery modelyou will be the leader of the clinical and IDT scope of practice includesclinical quality of care oversight provided by some of the comprehensive care team members : nurse practitioners licensed clinical social worker and medical have accountability for the comprehensive careprogramintegrity within include but are not limited to nurse practitioner quality of care oversightachieving program KPIswhichmaps to patient quality of careclinical education to team membersandbeing innovative when solutioning barriers to goalsof you will collaborate closely with theNurse Practitioner Call Center Manager having accountability for quality of careand education.

Travel will be required to all markets in the Northwest Regionwhich presently includes Salem Roseburg and MedfordOregon plus StocktonCalifornia.

This role requires a multifaceted approach encompassing all aspects of care delivery. Comprehensive geriatric assessments may be conducted in-office via telemedicine or through on-site visits at senior facilities patient homes dialysis centers and other appropriate collaboration with primary care physiciansisa mustto avoidfragmented care.

When performing patient visits you willmanagequality measures performaccurate risk assessments recommend adjustments to longitudinal careprovidecare fornon-life or limb threateningurgent concerns facilitateadvanced care planning andother key actions whilecollaborating with an interdisciplinary team to align care with patient health actionscombined withpreventive andtimelypatientcareaffordemergency department and hospital admissionavoidancewhich is a known factor in decreasing complicationswhilemaintaininga higherquality of lifein this subset of patients.

Essential Functions

  • Lead thecomprehensivecare team as a cohesive unit.
  • Work collaboratively withoperational partners.
  • Effectively manage clinical KPIs.
  • Serve as a positive and collaborative leader and team member.
  • Upholdhigh standardsof clinical excellence by thecomprehensivecare team and the call center.
  • Perform random case reviewsensuring integrity ofpatientcare.
  • Conduct comprehensive health assessments for P3 membersin a variety ofsettings.
  • Performtransitionofcarevisits toensure patient understanding of discharge instructions remove barriers for recovery andreduce avoidable hospital readmissionsor emergency department visits.
  • Perform comprehensive geriatric assessmentsusing soundclinical judgment.
  • Identifycare gaps and communicate recommended interventions to the members primary care provider and care team.
  • Triage and manage non-life or limb threatening conditions.
  • Collaborate with interdisciplinary teams using data-driven clinical tools to support coordinated value-based care.
  • Collaborate withthenurse practitionerswhenassistancein clinical decision making isidentified.
  • Communicateopportunities to advance the clinical model.
  • Educate P3team members when necessary.
  • Educatepatients their families and caretakerson health maintenance chronic disease management medications and preventive care.
  • Confidently use technology andevidence-based medicineto guide care planning and decision-making.
  • Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
  • Participate in quality improvement initiatives documentation audits and other activities that support clinical excellence and operational efficiency.
  • Maintain compliance with requiredtrainingstimelychart closures chart queries meeting attendancetimelyresponses toemailsand other requirements.
  • Maintain a license and DEAcertificatein good standing.
  • Rolemodelthe P3 Health Partnerscore values.

Qualifications

  • Active and unencumbered Oregon andCalifornia PhysicianLicensein addition to anystatewhere P3 Health Partnersoperates
  • Current DEAlicensein all states where you hold a medical license to practice
  • Minimum of 5 years medical director experiencepreferred
  • Geriatric Specialty certificationpreferred
  • Experience in home care or house call settings preferred
  • Experience in geriatric acute care
  • Ability to perform out-patient procedures which may include but are not limited to wound care management of skin tears and lacerations intravenous hydration phlebotomy urinary catheterization epistaxis management
  • Flexible self-drivencollaborativeand innovative personality
  • Travel

    Travel within the market for patient visits external to P3 facilities all Pacific Rim Regional Markets plus offsite meetings as requested

    Pay Range : $200K-250K
