CHARACTERISTICS OF THE WORK

Under the direction of the FHC Director of Population Health & Quality Improvement, assists Family Health Centers (FHC) patients in obtaining needed primary care, behavioral health, and other services after hospitalizations or emergency room visits.

Examples of Duties

EXAMPLES OF THE WORK (These statements are intended to be illustrations of the work of this class.)

Accesses and creates reports from external software programs to identify FHC patients who were recently hospitalized or who recently visited an emergency room.

Uses reports to identify patients utilizing emergency rooms for non-urgent care and connects them to FHC services.

Schedules patients recently discharged from hospital and / or emergency room departments for follow-up appointments with their primary care physician within 10 days of discharge.

Receives calls from the FHC Call Center to assist patients and hospital staff with follow-up care coordination.

Monitors the progress and outcome of services provided to FHC patients.

Makes referrals as needed to ensure patients receive appropriate care.

Participates in staff and community meetings.

Conducts outreach to patients due for preventative care screenings and follow-up appointments.

Identifies patients with Limited English Proficiency (LEP) and / or other communication barriers and uses appropriate interpretive service.

Performs related work as required.

Minimum Requirements

MINIMUM REQUIREMENTS

Bachelor's degree in Social Work, Nursing, Public Health, or a closely related field.

One year of experience working in a primary care or behavioral health setting.

An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Responds to patient and FHC needs via telephone, internet, fax, computer software, and in person.

Spends extended periods of time sitting and reading computer screens.
