IOP / PHP Group Facilitator SUD Outpatient Program

Tal Addiction Recovery & Behavioral Health is seeking a dedicated IOP / PHP Group Facilitator - Clinician to provide structured group therapy, individual support, and case management services to clients enrolled in the Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP). The ideal candidate will facilitate therapeutic group sessions, provide clinical interventions, collaborate with an interdisciplinary team, and ensure proper documentation of client progress.

DUTIES, RESPONSIBILITIES, & ESSENTIAL JOB FUNCTIONS

Facilitate and co-facilitate groups for clients enrolled in the IOP and PHP.

Conduct assessments of new clients to determine appropriate treatment plans.

Provide individual counseling sessions to clients as needed.

Perform case management duties to support client needs.

Collaborate with the interdisciplinary team to enhance treatment planning and service delivery.

Provide clinical interventions and crisis management as necessary.

Maintain accurate and timely documentation (within 24 hours of service) of all client interactions, including time spent and services provided.

Ensure compliance with all policies, procedures, and regulatory requirements.

Engage in ongoing professional development and training as required.

Perform other duties as assigned by the Program Manager.
