Pennsylvania Licensed RN Care Navigator

A company is looking for a RN Care Navigator - PA (Remote).

Key Responsibilities

Manage in and outbound calls to provide exceptional service to members

Educate members on kidney health, dialysis, and transplant options while facilitating engagement in the Kidney Health Program

Collaborate with healthcare teams to coordinate care and address patient needs, ensuring compliance with regulations and policies

Required Qualifications

Active, unrestricted RN license required

Bachelor's degree required

CCM certification preferred

Three years of experience in case management preferred

Experience in a dialysis or transplant center preferred
