Physical Therapist (PT) — Home Health Weekend Admissions (PRN)

Position : Home Health Physical Therapist — Admissions / SOC

Schedule : PRN, Saturday & Sunday

Location : In-home visits (service area : KS / MO as assigned)

Compensation : $150 per Start of Care (SOC) (paid per visit)

Position Overview

We’re looking for a licensed Physical Therapist (PT) to complete weekend admissions / Start of Care (SOC) visits for Home Health patients. This PRN role is perfect for a PT who is confident with evaluations, enjoys independent work, and wants flexible weekend-only hours.

What You’ll Do

Complete Start of Care (SOC) / admission PT evaluations in the home
Perform comprehensive assessments (mobility, strength, balance, pain, safety / fall risk, functional status)
Develop and document a skilled plan of care, goals, and visit frequency recommendations
Provide patient / caregiver education and initiate home exercise and safety plans
Communicate findings and recommendations with the care team and ordering providers as needed
Ensure accurate, timely documentation per home health guidelines

Requirements :

Active Physical Therapist license in BOTH Kansas (KS) and Missouri (MO) (dual-state licensure required)

Valid driver’s license and reliable transportation for home visits

Strong clinical decision-making, documentation, and communication skills

Ability to work independently and manage time efficiently

Preferred (Not Required)

Home Health experience , especially with admissions / SOC documentation

Compensation Details

$150 per SOC / admission visit (paid per visit)
