Social Service Assistant

Our Social Services Assistant will work directly with our Social Services Director.

Reports To : Social Service Director

Social Services Assistant Requirements : Must have a minimum of one year experience in long term care, mental health, or human services. Social Services Designee Certificate preferred.

Job Summary : To participate in a multidisciplinary team approach to provide for the psychosocial welfare, serve as a primary advocate, and meet the psychological and mental health needs of each resident to improve or maintain their highest practicable resident physical, mental, and psychosocial well-being. Ensure Federal, State, local, and facility regulations, guidelines and standards are followed.

Duties for Social Services Assistant, include but are not limited to :

1) Monitor patients in order to detect unusual behavior, and report observations to professional staff.

2) Provide mentally impaired or emotionally disturbed patients with routine physical, emotional, psychological or rehabilitation care under the direction of nursing and medical staff.

3) Record and maintain records of patient condition and activity

4) Organize, supervise, and encourage patient participation in social, educational, and recreational activities.

5) Interview patients upon admission and record information.
