[job_card.job_description]Collaborate with interdisciplinary teams to ensure comprehensive care delivery. Build and sustain relationships with community organizations to support patient advocacy. Connect patients with mental health and public health resources in the community. Maintain accurate patient records, treatment plans, and progress reports. Conduct assessments to track patient improvement and adjust interventions as needed. Prepare detailed case summaries and discharge plans. Education : Master’s Degree in Social Work, Counseling, or a related field. License : Active LPC or LMSW licensure. Experience : Minimum of 3 years working with the population served, including experience in : Delivering individual and family therapy. Providing crisis and behavioral interventions. Expertise in evidence-based practices and psychosocial assessment. Strong treatment planning skills with the ability to adjust plans as necessary. Excellent communication skills for patient counseling and interdisciplinary collaboration. Proficiency in crisis stabilization and emergency decision-making. What You’ll Do
Comprehensive Patient Care
- Conduct thorough mental health assessments and diagnostics to identify patient needs.
- Develop, implement, and evaluate individualized clinical treatment plans.
- Provide outpatient therapy tailored to adults and / or children with diverse mental health needs.
- Offer crisis intervention services, ensuring patient safety and immediate support.
