New Mexico Licensed Bilingual LPN

A company is looking for a Bilingual LPN Care Coach.

Key Responsibilities :

Utilize specialized care management software to call Medicare patients with chronic conditions monthly

Build rapport with patients to coach them on health improvement through SMART goals and self-management education

Implement and update the Plan of Care, including medications, appointments, and preventive care measures

Required Qualifications :

Fluent in both English and Spanish

Current, unrestricted LPN Compact License or New Mexico LPN license

3-5+ years' experience as a nurse

Proficiency with EPIC electronic health record and web-based applications

Self-directed and able to work independently while meeting performance metrics
