Role Summary

Our Licensed Practical Nurse (LPN) serves a pivotal role in our efforts to provide compassionate, top-notch care for our patients in the comfort of their place of residence. By personifying our Mission and Values, our Licensed Practical Nurse (LPN) acts as a bridge to independent living for our clients who may also require nursing care, under the supervision of a registered nurse, coordinate care with the interdisciplinary team, clients and their families, and the referring Agency.

Patient Population : Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.

Essential Functions :

• Promote / exemplify the Company's mission, vision, and values at all times.
• Under the direction of the RN, assist in identifying the client's physical, psychosocial, and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, and evaluations.
• Participate in planning and implementing care with the RN through the Plan of Care (POC). Assist clients in recovering or maintaining life functions; prevent illness and relapse.
• Provide care utilizing infection control measures that protect clients and staff (OSHA).
• Assure the continuity of care through the delivery of quality patient care.
• Provides effective communication to clients, their families, team members, and other health care professionals, and regular participation in Case Conference / Team Meetings.
• Monitors assigned cases to ensure compliance with the requirements of third-party payers.
• Completes appropriate documentation promptly to ensure compliance with company policy.
• Demonstrates commitment and professional growth by participating in in-service programs and maintaining / improving competency.
• Meet mandatory continuing education requirements of the Agency / licensing board.

On-going employment as an LPN requires maintaining a valid LPN license in the state where the nurse practices in and a valid CPR certificate at all times. The employee is responsible for renewing these before they expire to continue employment.
