[job_card.job_description]This position is a Support physicians and the interdisciplinary team to facilitate patient care and optimize clinical outcomes, patient satisfaction, and cost efficiency. Manage utilization review, care coordination, patient education, and discharge planning for an assigned caseload. Facilitate precertification, authorization, and communication with payors. Identify and remove barriers to timely care and reimbursement across the continuum. Apply process improvement methodologies to evaluate and enhance care outcomes. Coach and support clinical documentation accuracy; serve as a resource for coders to ensure documentation reflects severity of illness. Coordinate communication with physicians and clinical teams. Demonstrate knowledge and competencies appropriate for all patient populations served. Experience with CarePort required. Preferred : Experience charting in AllScripts, GEMS / Sunrise; experience in utilization management, discharge planning, performance improvement, or managed care reimbursement. Strong communication, negotiation, analytical, and organizational skills. Ability to work independently and exercise sound judgment when interacting with physicians, payors, patients, and families. Benefits are available to full-time employees after 90 days of employment. A 401(k) with a company match is available to full-time employees with 1 year of service on our eligibility dates.

BSN required.
Active RN license in the State of Michigan required.
BLS certification through AHA required.
Minimum 5 years of acute inpatient case management experience
