This position is a FULLY ONSITE position, Monday through Friday 8am to 5pm.

The Quality Coordinator will assist the Organizational Director of Quality and Senior Quality Coordinator in quality and safety improvement in the acute care setting of Stamford Health. Reporting to the Director of Quality, this position will provide support for organizational quality related data collection, submission, analysis, and reporting of clinical quality measures and initiatives. The position will work closely with clinical departments, interdisciplinary teams, and organizational committees.
Supports the abstraction, monitoring, analysis, and reporting of various prospective & retrospective measures and clinical quality data from specific standardized and customized quality databases across the organization.Monitors and collects data for clinical indicators and utilization as defined by but not limited to : Get with the Guidelines, STS, VON, CMS, DPH, TJC and required internal data elements that support PI initiatives.Assists in monitoring and collecting data for Leapfrog, Q-HIP and other payor quality programs.Supports HAC and PSI reduction through participation in improvement activities and PRSM committee.Supports the maintenance of a unified organizational report of quality and safety indicators from across the organization.Develops and maintains competence in standardized quality measure specifications and benchmarks for various public reporting and pay for performance program measures.Supports data collection for quality improvement projects as requested.Audits data in an accurate and timely manner to ensure report accuracy.Performs qualitative clinical and medical record review and evaluation on a case-by-case basis.Supports outcome monitoring and submission within external and internal databases.Provides support and coordination for assigned QI Committees.Maintains knowledge of data definitions and data entry guidelines.Support identification of desirable and undesirable trends in quality and safety measures.Participate in quality and safety related data analyses.Support submission of required quality and safety measures such as Joint Commission ORYX / Core Measures, electronic quality measures, and Centers for Medicare and Medicaid Services quality reports.Assist with organizational compliance with the Joint Commission Performance Improvement Chapter and associated standards.Support retrieval, validation, and distribution of CMS reports.Bachelor’s degree in nursingA Master’s degree is preferred.Connecticut State RN License requiredA minimum of 3 years of acute care clinical experience is preferred.Two or more years of quality or safety experience is preferred.Data abstraction experience is preferred.Proficiency with the use of electronic health records is required.High level of competency with Microsoft Office or equivalent is preferred.A willingness to learn new tools and techniques is required.Strong organizational and interpersonal skills are required.
