[job_card.job_description]Physician notification to obtain necessary orders for intervention(s) per state regulations Changes in patient / client condition Adheres to Maxim's policies and procedures and applicable protocols Identifies and prioritizes actions based upon patient / client care requirements Assures patient safety at all times and is the recipient of services provided Verifies that documentation reflects patient / client collaboration, participation in plan of care, expected patient / client outcomes, and selected nursing interventions Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies, and patient / client care outcome standards as applicable. Monitors and documents the effectiveness of interventions, treatments, and medications Evaluates patient / client and provides a safe, comfortable, and therapeutic patient / client environment Monitors the safety and effectiveness of the environment and equipment, including universal precautions Participates in activities to facilitate good interpersonal communications Communicates observations and significant changes in patient / client status to the Director of Clinical - - Services and / or Clinical Supervisor / Case Manager or licensed designee Documents all physician collaboration / coordination of care in the medical record and Attends case conferences, team meetings, and education in-services as required Nurse Practice Act Standards of Nursing Practice Standards of Patient Care Standards of Accrediting Body (ACHC, Joint Commission, etc.) State and Local Laws Patient Bill of Rights Maxim Healthcare, Nursing, and Client’s Hospital Policies Performs other duties as assigned Graduate of an accredited school of nursing License Required 3.0 GPA preferred Complies with accepted professional standards and practice Current Hands-On BLS card PPD or chest X-ray, if applicable Successful completion of the Maxim Homecare Novice Nurse Program orientation and training Proficiency in the English language is required Excellent communication skills Technological proficiency, including mobile devices for documentation Customer First Mentality - every decision we make should be made through the lens of the customer. RYZ Labs is hiring a Novice LPN - Private Duty to provide services to patients / clients in their places of residence.
Responsibilities :
- Utilizes the nursing process to plan, implement, and evaluate patient / client care and / or contracted services
- Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders
- Identifies and documents changes in the patient’s both physiologic and psychosocial condition
Notifies Director / Supervisor if applicable on the following;
communicates this information as needed
