  • Job Summary :

  • The fundamental expression of that mission is the care of those individuals who avoid shelters and sleep on the streets.  This population suffers very high mortality rates and presents a complex burden of co-occurring medical, mental health and addiction problems, placing them at great risk.  As the Street Team Care Coordinator, you will be part of a multi-disciplinary primary care team and will provide complex case management for a panel of adults with a history of chronic unsheltered homelessness.
  • Some examples of complex case management that you will provide include appointment coordination, access to transportation and benefits, connection to housing and legal resources, and mitigating other barriers to health care.
  • Within your panel of patients, a subset will be eligible to be part of a MassHealth initiative that provides qualified MassHealth enrollees with a service benefit called Behavioral Health Community Partners.
  • To succeed in this role, you must be a flexible, compassionate, and self-motivated individual who embraces the mission.  You must be committed to delivering low-threshold, high-quality care coordination in various non-traditional locations, to a vulnerable population of individuals with a history of chronic street homelessness.
  • Because you’ll be practicing on the streets, you must be able to walk multiple miles a day in varying weather conditions.            Responsibilities :

  • Provide outreach-based case management, and non-judgmental support, in collaboration with other Street Team members, and as part of the Behavioral Health Community Partners Program.
  • Engage patients with substance use disorder and mental illness, with a strong commitment to trauma-informed care and harm reduction practices.
  • Complete health / needs assessments and develop integrated care plans with the patient and primary care team.
  • Assess social determinants of health and address these needs according to the patients’ goals.
  • Track appointments, referrals, and primary care activities and follow up with the patients to ensure that they keep their appointments or reschedule if the appointment is missed.
  • Participate in team case conferences and advocate for patients’ needs.
  • Perform street outreach and home visits with Street Team as necessary.
  • Qualifications :

  • Experience working with vulnerable populations, including persons with a history of trauma, those experiencing socioeconomic stress, homelessness, mental illness, and substance use disorders.
  • Strong interpersonal skills and case management problem-solving.
  • Efficient, organized, detail-oriented, and able to complete tasks in a time-sensitive manner.
  • Self-directed with the ability to work both independently and collaboratively with clinical staff on shared patient goals.
  • Associate or bachelor’s degree in a related field and / or equivalent two years relevant human services experience.
  • Compensation and Benefits :

  • The compensation increases based on years of experience and ranges from $22.07 - $35.31 hourly.
  • BHCHP full-time employees are eligible for our competitive time off program, health, dental and vision insurance, 403B retirement savings plan, pre-tax MBTA pass program with 40% discount, additional compensation for demonstrated bilingual proficiency and more.
