THERAPIST / CASE MANAGER

JOB DESCRIPTION

General Description : The Therapist / Case Manager provides therapeutic, case management, aftercare, and cross-system coordination services to a caseload of approximately 10 children.  Provides guidance and leadership to the treatment team regarding clinical and milieu issues.  Performs other duties as assigned and required by the Director of Clinical Services..

FLSA Status : Exempt. Minimum 40 hours per week; additional hours and on-call work as needed or required.

Supervisor : This position with report to the Director of Clinical Services

JOB RESPONSIBILITIES :

• Provides weekly individual, group, and family therapy for assigned cases; documents goals and progress in each child's file.
• Handles case management and aftercare services for assigned cases.
• Maintains contact with family, agency representatives, Department of Family and Protective Services caseworkers, court advocates, and other community resources.
• Maintains and supports the child’s school, medical, dental, behavioral health, and other service needs.
• Participates in team meetings to keep team informed of daily issues; keeps significant parties informed of child's needs, problems, and progress; schedules visits and supervises visits as needed; and makes proposals for child's medical, emotional, and therapeutic needs.   Uses client information system.
• Oversees clinical intake and discharge for assigned caseload; completes preliminary treatment plan and diagnostic assessment, or new intakes assigned to the caseload;
• Facilitates intake interviews and placement of children;
• Develops individualized treatment plans for new intakes for each client;
• Oversees ongoing treatment and service planning for assigned cases;
• Participates in formulating individual treatment changes, plans discharges, and arranges for  special therapy or treatment needs;
• Coordinates and leads treatment plan reviews;
• Attends court reviews and Department of Family and Protective Services meetings;
• Testifies in court hearings; and presents cases to psychiatric and psychological consultants.
• Provides direct care and transportation of children as needed.
• Provides in-person, phone or video conference consultations in the event of a crisis, as needed.
• Uses authorized management of aggressive behavior techniques as needed.

Maintains appropriate licensure through required annual training and continuing education.

Assists in the orientation and training of new employees; and assists in the presentation of in-service training when assigned.

PHYSICAL REQUIREMENTS :

Prolonged periods sitting at a desk and working on a computer.

Must be able to lift up to 15 pounds at times.
