Position Overview :

Navitas Healthcare, LLC is seeking an Associate Transplant Coordinator for a job in New York, NY.

Responsibilities :

• Support coordination of care for kidney transplant patients , ensuring timely follow-up and communication.
• Assist with reviewing medical records, procedure codes, and required documentation.
• Communicate with insurance providers, government programs, and third-party agencies to obtain necessary authorizations.
• Maintain accurate records, logs, and patient information using Microsoft Word and Excel.
• Provide administrative, scheduling, and patient-support functions for the transplant service line.
• Collaborate with clinical teams to ensure smooth workflow and timely care coordination.
• Interact with patients and families, offering clear communication and excellent customer service.
