Social Worker (Hospice) Part Time El Paso

We are currently seeking qualified candidates for a part time Social Worker position in the El Pasoarea.

Job Description :

Providing social service activity as it relates to the patients and their families. Thisincludestreatment planning and consultation with other staff members. Experiencein assessing the patients and their familys emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patients current statusto be retained in the patients chart. Participate in discharge planning and in-services programs related to the needs of the patient.

Qualifications :

• LicensedSocial Workerin the State of Texas - required
• 1 yearsexperience with license - required
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Clean backgrounddrug screen and driving record
