Job Details

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with Service Excellence Standards and Core Values.

Works with staff in development of professional skills, new programs, patient treatment plans as needed.

Treatments are performed as ordered in a safe, timely manner with involvement of staff and family members when indicated.

Medical documentation is in accordance with APTA standards of care.

Attends and takes part in services, meetings, and educational sessions.

Assists in adjusting schedules to accommodate changes in caseload in the department.

Ensures an orderly, clean and safe work area.

Evaluates patients referred for physical therapy using specialized testing procedures and skills, incorporating data from medical records and assessing strength, range of motion, coordination, perception, sensation, ambulation, transfers, bed mobility, and rehabilitation potential.

Correlates data from evaluation to determine individualized physical therapy plan of care directed towards short and long term goals.

Supervises operation of specific treatment team and meets regularly with PTA to discuss patient and set goals.

Performs evidence-based care and seeks out research to support interventions.

Respects confidentiality of patient records and facility operations, using discretion when discussing patient matters.

Adheres to all facility and corporate policies and procedures.

Job Requirements

BLS certification (AHA only) required for onboarding.

License / Certificate Verification required for onboarding with primary source verification and state-specific registry.

Proof of MMR immunizations or titer with lab printout required for onboarding; no declinations accepted.

Proof of Hepatitis B vaccinations or titer with lab printout required for onboarding; no declinations accepted.

Driver's License or State ID required for onboarding.

Proof of 2 Step PPD or IGRA (Quantiferon / Tspot) required for onboarding with specific timing requirements.

Proof of Varicella immunizations or titer with lab printout required for onboarding; no declinations accepted.

Social Security Card required for onboarding.

Criminal Background Check results required for onboarding including multiple searches done within 60 days of start and updated annually.

Employment Verification for 5 years required for onboarding.

Drug screening (10 panel) completed within 30 days of start required for onboarding.

McLeod Profile (resume or profile) required for submission.

McLeod Skills Checklist (clinical only) required for submission and updated annually.

McLeod Confirmation of Assignment form required for onboarding.

Schedule Information

Adjusts schedules to accommodate changes in caseload in the department.

Additional Details

COVID-19 Vaccine is optional with medical / religious exemptions considered case by case with physician's statement or signed traveler statement.

Fit Test required for travelers with make / model to be completed within a year of start (optional).

Influenza Vaccine required October - April with medical / religious exemptions considered case by case with physician's statement or signed traveler statement (optional).

Covid Test optional.

Annual Negative PPD optional if 2 step PPD not done within past year.

Annual TB Questionnaire optional.

Positive PPD with Negative Chest X-Ray optional with X-ray completed within 90 days of start and uploaded with positive test results.
