The Licensed Vocational Nurse (LVN) plays a vital role in delivering direct patient care under the supervision of a Registered Nurse. You will follow care plans, administer treatments, and collaborate effectively with the healthcare team to ensure excellent patient outcomes.
Key Responsibilities :
- Delivering patient care in accordance with the state Nurse Practice Act and agency policies.
- Implementing care plans developed by Registered Nurses.
- Maintaining accurate, timely documentation aligned with care plans.
- Providing education and information to patients and families based on diagnoses and care plans.
- Coordinating home health services and communicating patient needs to appropriate disciplines.
- Following all infection control protocols and safety guidelines.
- Utilizing medical equipment and supplies effectively.
- Engaging in personal and professional development.
- Adhering to agency standard operating procedures for documentation submission.
Communication :
Preparing clinical notes and updating primary physicians as needed.Communicating changes in patient conditions to physicians and obtaining necessary orders.Coordinating care with the Clinical Team Manager and Clinical Team Coordinator.Additional Duties :
Participating in on-call duties as needed.Understanding and observing Patient Rights and Privacy Practices.Ensuring equipment and other necessary items are accessible for patient care.Maintaining a manageable patient case load and timely documentation.Being available for patient visits during business hours (8 am - 6 pm).Updating time cards daily per policy.Contacting patients the evening before scheduled visits to confirm arrival times.Seeking pre-approval for any overtime from the supervisor.Demonstrating reliability in attendance and punctuality.Protecting the privacy of all patients and agency information.Exhibiting flexibility and a positive attitude while working with others.Communicating clearly and cooperatively at all times.Upholding the agency's professional image by adhering to dress code.Performing other related duties as assigned.We offer recognition and rewards for your performance in a supportive environment that promotes growth. Required Qualifications :
High school diploma or equivalent.Graduate of an accredited practical or vocational nursing program.Current unrestricted license as a LVN in Texas.Current CPR certification.A valid Texas Driver's License and personal auto insurance as per organizational guidelines.A minimum of 2 years of clinical experience in home health or medical / surgical settings.Strong understanding of nursing practice standards and regulations related to home care.Tuberculosis screening and proof of immunity to specific diseases.Skills / Abilities :
Excellent communication skills, both verbal and written.Ability to practice independently and deliver nursing care effectively.Physical capability to lift, transfer patients, and handle necessary equipment.Basic computer skills, including Microsoft Word and email applications.Strong organizational skills and capacity to prioritize tasks.Experience with electronic medical records.Ability to work collaboratively in a multidisciplinary team setting.Physical and Mental Requirements :
Ability to lift up to 50 pounds.Capacity to push or pull objects weighing up to 50 pounds.Ability to stand and sit for extended periods of time.Capability to effectively drive and operate a vehicle.Ability to follow both verbal and written instructions.Strong reasoning skills for navigating simple and complex situations.Work Environment :
This position involves working in patient homes, which may have varying conditions, including exposure to odors, pets, and potential health hazards. There may be instances of challenging interactions with patients or family members. The ability to remain calm and courteous in stressful situations is essential.
Our compensation package includes a competitive hourly pay range of $20.00 to $35.72 based on experience and qualifications, along with a comprehensive benefits package and opportunities for advancement.
