Position Requirements : Active, unencumbered license in WI Minimum of 1-3 years of relevant experience, depending on specialty Current Certifications : BLS and / or ACLS, plus any specialty-specific requirements Experience with EMR systems (Epic, Cerner, as applicable) Job Responsibilities :
- Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care
- Identify teaching needs and goals related to patient diagnosis through collaboration with patient / family and interdisciplinary team
- Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences
- Document all assessments, interventions and patient responses accurately and timely in medical records
- Participate in patient rehabilitation program on assigned unit utilizing nursing process of assessing, planning, implementing and evaluating
- Provide direct patient care with priority on education of patient / family / significant others based on identified healthcare needs
- Initiate discharge planning on all patients from day of admission and provide patient teaching in anticipation of discharge
- Communicate pertinent information regarding changes in patient status to ensure continuity of care and patient safety
- Provide direction, training and supervision of ancillary nursing personnel including Licensed Practical Nurses and Certified Nursing Assistants
- Delegate appropriately and utilize problem solving techniques in decision making and priority setting
- Ensure personal and environmental safety in accordance with hospital and nursing policies and procedures
- Contribute positively to unit functioning by providing calm direction to staff, patients and families
- Maintain communication with manager and senior nursing staff regarding service needs including supplies, equipment and clinical training
- Provide cost effective care while promoting quality patient outcomes and monitor supplies and charges
- Assume charge responsibility following orientation period as requested
- Pursue professional development through continuing education and self study
- Prioritize activities and plan for effective use of time and staff to meet patient needs
- Maintain current BLS certification
- Follow all hospital guidelines for personal appearance, attendance and punctuality
