Job Title : Registered Nurse – Home Health

Shift :

8-Hour Day Shifts
Monday - Friday

Location : Huntsville, AL

Guaranteed Hours : 40 Hours / Week

Position Summary :

Seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality care to patients in their homes. The RN will coordinate care with interdisciplinary teams, assess patient needs, and implement personalized care plans. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in a home setting.

Key Responsibilities :

Assess patients in stable and emergent home health situations.

Develop, implement, and evaluate individualized Plans of Care (POC).

Document all patient interactions, changes, and care plans accurately.

Educate patients and families on medications, diet, safety, and self-care.

Supervise and guide HHAs, STNAs, and LVNs to ensure continuity of care.

Provide pain management and educate patients / families on techniques.

Coordinate referrals and ensure compliance with third-party payers.

Participate in on-call rotations and interdisciplinary team meetings.

Maintain infection control standards and ensure safe use of medical equipment.

Required Qualifications :

Education : Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program

Licensure & Certifications : Current RN license in the state of practice

Current CPR certification

Valid driver’s license and reliable transportation with insurance

Experience : 1–2 years as an RN in a clinical care setting

2+ years of home health experience

Technical Skills : Proficiency with Homecare Homebase software

Experience with OASIS documentation

Competency in IV care, wound care, phlebotomy, and catheter maintenance

Preferred Qualifications :

Education : Bachelor of Science in Nursing (BSN)

Certification : CHPN (Certified Hospice and Palliative Nurse)
