What You Should Know About the LPN Team Float Role :

• This is a DRIVING position and will travel to see patient in both homes and facilities within the Wilmington & Washington Courthouse coverage area
• This is a FT position, M-F day shift with an every 6th weekend rotation and 1 holiday per year
• We provide superior care and superior services to patients at their end of life journey . Only those who have a heart for hospice will succeed

The LPN / RN Float Essential Duties :

The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.

Qualifications :

LPN's - Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience.
