Minnesota Licensed Clinical Pharmacist

A company is looking for a Clinical Pharmacist (MN License Required).

Key Responsibilities

Review and interpret patient care notes for prior authorization approvals and care plan recommendations

Monitor patient adherence and evaluate therapies for side effects or medication-related issues

Supervise communication of support personnel and maintain compliance with HIPAA and pharmacy regulations

Required Qualifications

Doctor of Pharmacy (PharmD) or equivalent advanced degree required

Must hold a valid Pharmacist license in Minnesota

1+ years of experience preferred but not required

Knowledge of complex / chronic disease states and therapies preferred

Commitment to obtaining any additional licensure as required
