Senior LTSS Service Care Manager (RN)

Position Title: Senior LTSS Service Care Manager (RN)
Work Location: Member-Facing (Laredo TX)
Assignment Duration: 6 Months with a possibility of extension or conversion
Work Schedule: M-F 8-5 with possibility of OT when warranted

Key Responsibilities:

• M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members homes to complete assessments/documentation
• Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, completing outreaches
