What are the Minimum Skills, Experience and Educational Requirements?
- RN with Michigan license, Masters degree in Nursing required with Advanced Practice Certification.
- Current BLS certification.
- Valid Michigan driver's license.
- Experience in Internal Medicine or family Practice required, Hospice and / or geriatric experience preferred.
- Certification in Hospice and Palliative Care preferred or willing to obtain in 3 years.
- Must be able to manage several job tasks simultaneously and be able to function effectively under high stress situations.
- Ability to read and interpret data from equipment and computer, including data entry.
- Good communication skills.
- Able to control confidentiality of sensitive information.
- Capable of working independently and making judgments on the basis of collected data.
- Capable of developing effective working relationships.
- Knowledge of billing and ICD-9 HCPCS coding is preferred.
- Ability to be mobile to carry out responsibilities throughout the community.
- Knowledge of Hospice philosophy, Medicare Hospice benefit and end of life care.
- Knowledge of Hospice Medicare conditions of participation
What are the Essential Job Functions and Responsibilities?
Clinical Responsibilities :
Performs a comprehensive assessment / history and physical including subjective and objective data obtained from all relevant sources.The assessment is accomplished using appropriate assessment techniques, including ordering and interpreting diagnostic tests.The assessment should include the use of the standardized prognosis guidelines for determining Hospice eligibility.Collaborates with the medical director in the development of the plan of care.Patient's responses are assess based on expected outcomes consistent with scientific and practice knowledge as well as the patient situation.In collaboration with the medical director interventions are changed based on the patient's response Makes home visits as needed.Makes rounds in the Hospice Residence / Hospital as required.Completes, reviews and signs documents timely to meet regulatory requirements, including face to face visits and is available for support and consultation during normal office hours and after hours.Leadership :
Demonstrates accountability for personal, professional, ethical and legal standards.Assists in the development and review of clinical protocols.Promote open communication across the continuum. Acts as a liaison to physicians in the community. Promotes Hospice and Palliative Care through personal educational contact with health professionals and the community.Professional Development :
Assumes responsibility for continually improving own expertise.Participates in continuing education and in-service programs related to Hospice and Palliative Care and other pertinent topics.Performance Improvement :
Participates in the performance improvement program.Coordinates with the Quality and Compliance RN to meet all regulatory requirements.Participates in committee activities.Regular attendance at standing and ad hoc meetings to include :
Community Hospice and Hospice House Team meetings as availablePalliative Care meetingsAd Hoc meetings with Medical Director and ManagerRelevant community meetings and conferencesStaff meetingsWhat are the Critical Demands of the Job?
