[job_card.job_description]Work-Life Balance : No nights or weekends, 4 day work week with flexible schedule Provider-Focused & Strong Clinical Support : Administrative, coding, and billing support provided, with dedicated medical assistant and dictation support available Professional Training & Growth : Ongoing functional medicine training, opportunities for career advancement, and support for continuing education Compensation & Benefits : Comprehensive benefits package (Medical, Dental, Vision, Life Insurance), 401(k) with 4% company match, up to 4 weeks PTO, and malpractice insurance covered by OnePeak Medical Provide direct patient care through office visits, telehealth, and occasional same-day appointments Perform comprehensive assessments, order and interpret labs / imaging, and diagnose acute and chronic conditions Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions Educate and empower patients on preventive health and functional medicine principles Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff Document visits in the EHR (NextGen) in a timely and compliant manner Participate in team meetings, quality improvement initiatives, and training sessions Support clinic operations by contributing to a positive, team-oriented environment Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain) Board certification in Family Medicine, Internal Medicine, or relevant specialty Minimum 2 years of clinical experience preferred (functional / integrative medicine experience a plus) Strong interest in preventive, functional, and lifestyle medicine Excellent communication and patient relationship skills Comfortable working in a fast-paced, collaborative environment Proficient with electronic health records (NextGen experience a plus) \tComprehensive primary care for men and women \tAnnual wellness exams and preventive screenings \tNutrient IV therapy and targeted injections \tHormone testing and optimization, including bioidentical hormone therapy \tAutoimmune testing and personalized treatment strategies \tNutrition and weight management consulting \tPlatelet-Rich Plasma (PRP) therapy \tAesthetic and anti-aging treatments \tNatural, evidence-based nutritional supplements \t...and a growing range of services designed to support vibrant, healthy living \tComprehensive primary care for men and women\r\n \tAnnual wellness exams and preventive screenings\r\n \tNutrient IV therapy and targeted injections\r\n \tHormone testing and optimization, including bioidentical hormone therapy\r\n \tAutoimmune testing and personalized treatment strategies\r\n \tNutrition and weight management consulting\r\n \tPlatelet-Rich Plasma (PRP) therapy\r\n \tAesthetic and anti-aging treatments\r\n \tNatural, evidence-based nutritional supplements\r\n \t...and a growing range of services designed to support vibrant, healthy living\r\n\r\nAt OnePeak Medical, we are working to transform healthcare into a system that values innovation, prevention, and patient empowerment.

Compensation : Guaranteed salary with earning potential of $300k+, relocation stipend offered!
