Overview

Job Title : Primary Care Physician

Location : Guilford, CT

Work Schedule : M-F, very light call

Scheduled Hours : 40, 36 clinical / 4 admin, 4 day work week available

Position Type : FTE

EMR System : EPIC

  • 16-20ppd with light, shared phone-call responsibilities with regional group

Responsibilities

  • Evaluate patients, and treat acute and chronic illnesses
  • Order, perform and interpret diagnostic tests
  • Involve patients and their families in care and treatment decisions
  • Create treatment plans and complete orders in patient's charts
  • Collaborate on improvements in care delivery and service with staff, other physicians and management
  • Initiate appropriate specialty referrals
  • Provide clinical information to the interdisciplinary team in a clear and timely mannerEnsure that proper patient documentation is timely, accurate and complete
  • Work with practice management to ensure proper compliance with billing, coding, and CPT code assignment
  • Achieve and maintain optimal patient access
  • Provide interdepartmental medical consultation as requested
  • Participate in the development of medical protocols, standard procedures and management of patient-care-related activities
  • Provide on-call coverage
  • Give feedback to and accept feedback from all staff in a respectful manner
  • Act in a positive and professional manner, complying with practice policies, procedures and federal and state laws
  • Perform other duties and responsibilities as assigned
  • Promote health education in conjunction with other health professionals

    • Qualifications

    Degree of MD or DO with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum)

    Licensure

  • Valid American Board certification in either internal medicine or family medicine and renewed (if needed) prior to expiration
  • Valid and unrestricted Connecticut medical license
  • BLS certification
  • Valid DEA registration and Controlled Substance Certificate or in the process of obtaining
