About The Position

Job Title : Physical Therapist Profession : Therapy / Rehabilitation Specialty : PT Home Health Duration : 13 weeks Shift : Day Hours per Shift : 8 Experience : Minimum of 2 years of experience required License : NH state license required; will accept subs with license pending if able to obtain by the start date Certifications : BLS, Heart Saver CPR equivalent, Manual Therapy Must-Have : Travelers are accepted; local candidates located within 50 miles will receive a rate decrease. Description : The position involves providing physical therapy services in patients' homes. The therapist will be responsible for conducting assessments and creating individualized treatment plans. Visits will be made to approximately 6 patients daily. Patient demographics include adults and geriatric populations with various acute, subacute, and chronic diagnoses. Therapists will provide interventions focused on fall risk reduction, strength training, and gait training. Collaboration with a multidisciplinary team is essential to achieving patient goals. Documentation will be completed using an electronic health record system. Therapists must be skilled in using adaptive devices and equipment as needed for patient care. The role requires time management skills to efficiently manage patient visits. Applicants should have experience in home health or a similar setting. Additional skills such as critical thinking and effective communication are required for this position. Weekend work may be occasionally required depending on patient needs. There are no call requirements for this position. Travelers should be aware that former staff are not accepted for travel roles. References will be required prior to an offer. A background check will be conducted as part of the hiring process. Drug screening is required within the specified timeline before starting the position.
