Job Summary
Under the physician’s orders, the RN Case Manager is responsible for assessing, identifying, and coordinating the needs of terminally ill patients and their families. The RN provides preventative and supportive care, ensuring comfort and enhanced quality of life. This role requires strong assessment, coordination, teaching, and critical-thinking skills while working collaboratively within an interdisciplinary team.
Responsibilities
- Conduct patient assessments including physical, psychosocial, spiritual, and age-specific evaluations; initiate appropriate referrals and interventions.
- Serve as the primary RN Case Manager, providing continuity of care and coordinating services aligned with patient / family goals.
- Develop, implement, and update the Plan of Care (POC) in collaboration with the patient, family, interdisciplinary team (IDT), attending physician, and partnering facilities.
- Identify learning needs, barriers, and preferences of patients and caregivers; deliver tailored education and follow-up.
- Promote a culture of safety; apply best-practice safety assessments, critical thinking, and ethical judgment in all clinical situations.
- Follow infection control protocols including standard precautions, proper PPE use, and hand hygiene; provide related patient / family education.
- Participate in IDT meetings, contributing to care planning and interdisciplinary collaboration.
- Maintain documentation that meets organizational standards for accuracy, productivity, timeliness, and compliance.
- Provide support to families following a patient’s death, coordinating closure processes with the IDT.
- Supervise and direct care provided by LPNs and Home Health Aides; complete supervisory visits and evaluations per policy.
- Maintain thorough knowledge of agency policies, clinical procedures, state / federal regulations, and Joint Commission standards.
- Participate in 24 / 7 patient care coverage as part of team responsibilities.
- Educate community members on hospice philosophy when assigned (case consultations, in-services, seminars).
- Provide nursing education and support to IDT members.
- Administer, monitor, and document medications according to physician orders; use age-appropriate care practices.
- Attend required trainings, skills reviews, and continuing education programs.
- Build and maintain professional relationships with patients, families, facility partners, and physicians.
- Maintain current credentials, CPR certification, licensure, TB testing, and continuing education.
- Perform additional duties as assigned by the supervisor.
Qualifications
Active Registered Nurse (RN) license (state-specific)Minimum 1–2 years of clinical nursing experience; hospice experience preferredStrong assessment, communication, and care coordination skillsAbility to work independently and within an interdisciplinary teamKnowledge of hospice care philosophy and end-of-life care practicesValid CPR certification and up-to-date clinical credentialsReliable transportation and ability to travel as neededCompany Description
