• Number of Physicians in Practice : 1 remaining physician
• Hem / Onc only practice
• Adults only
• No Radiation Oncology on-site, but connection to Rad Onc team at Loyola University Medical Campus, which is 5 miles from practice.
• No APP's in practice
• Documentation System / EMR : epic
• Estimated Credentialing Timeframe : 3 months
