Title : Case Manager

Sign on Bonus : Sign on Bonus of $600 (Half paid out at 3 months and the other half at 12 months).

Salary Range : $24.04 - $26.44 / hour, $50,000.00 - $55,000.00 / annually

We are currently seeking a compassionate and organized Case Manager to join our team in providing critical support to youth in shelter programs.

Key Responsibilities :

  • Coordinate the successful entry and transition of youth into and out of shelters.
  • Ensure compliance with court orders and assist clients in maintaining law-abiding behavior.
  • Meet frequently with clients and families to monitor progress in shelter, school, and community settings.
  • Develop comprehensive case and transition plans tailored to client and family needs.
  • Collaborate with community agencies to provide appropriate referrals and support services.
  • Maintain accurate and timely documentation of client interactions and service coordination.
  • Support a safe, clean, and well-maintained shelter facility.
  • Provide work direction to staff, assist with training, and maintain a positive work environment.
  • Ensure compliance with all regulatory and licensing standards.
  • Qualifications :

  • Bachelor's degree (B.A.) in a related field or equivalent combination of education and experience.
  • One to two years of related experience, preferably working with youth or in shelter programs.
  • Strong organizational and time-management skills.
  • Excellent verbal and written communication skills.
  • Cultural competency and trauma-informed care experience are highly valued.
  • Proficiency in Microsoft Office Suite and basic computer skills.
  • Valid driver's license required.
  • Work Environment :

  • Ability to work evenings, holidays, and weekends as needed.
  • Moderate noise levels in a dynamic, team-oriented environment.
  • Physical Demands :

  • Regularly required to stand, walk, and communicate effectively.
