LVN / LPN / Ambulatory Care

Key Responsibilities :

• Provide direct patient care including taking vital signs, administering medications, and assisting with daily living activities.
• Monitor and document patients' health status, reporting any changes to the supervising RN or physician.
• Maintain accurate and timely patient records in accordance with facility and regulatory standards.
• Assist in developing and implementing patient care plans.
• Provide emotional support and education to patients and their families.
• Ensure compliance with infection control and safety standards.
• Collaborate with other healthcare professionals to promote a safe and therapeutic environment.
