Physical Therapist (Home Health) Job Summary

We are seeking a licensed Physical Therapist to provide home health physical therapy services across multiple counties in West Virginia. This role focuses on patient-centered care, functional rehabilitation, and improving independence. No weekends or on-call shifts are required, offering a predictable weekday schedule.

Physical Therapist (Home Health) Key Responsibilities

• Perform comprehensive physical therapy evaluations per physician orders
• Review patient medical history, diagnoses, precautions, and contraindications
• Develop individualized treatment plans with measurable functional goals
• Deliver skilled physical therapy treatments in the home setting
• Reassess patient progress and adjust treatment plans as needed
• Communicate patient needs and progress with physicians, care teams, patients, and families
• Recommend and assist with adaptive and durable medical equipment
• Maintain accurate clinical and billing documentation
• Ensure compliance with HIPAA, company policies, and state / federal regulations
• Supervise Physical Therapist Assistants and Therapy Aides per state practice act
• Participate in staff development, training, and continuing education
