Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time, in-person role for an APRN or PA working independently in PALTC facilities.
- Manage comprehensive patient care by conducting exams, diagnosing, treating illnesses, prescribing medications, and providing preventive services.
- Work autonomously within scope, making clinical decisions and escalating as necessary, while collaborating with facility leadership and interdisciplinary teams.
- Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement initiatives.
- Promote health, safety, and infection prevention, educating staff and patients on wellness and disease prevention.
- Require at least 2 years of experience, board certification, and relevant degrees. Long-term care experience is preferred.
- Proficiency in EHR and telemedicine tools, strong communication skills, and the ability to work independently are essential.
