A company is looking for a GUIDE Program Coordinator to support caregivers through the onboarding process for dementia care services.
Key Responsibilities
Guide patients and caregivers through the onboarding process after enrollment
Monitor completion of dementia screening and health assessments
Coordinate tablet shipment and provide orientation to the GUIDE program
Required Qualifications
Bachelor's degree in healthcare administration, social work, public health, or related field preferred
Experience in patient navigation, care coordination, or healthcare customer service
Detail-oriented with excellent organizational and time management abilities
Comfortable working in a fast-paced, tech-enabled environment
Familiarity with CMS programs or dementia care is a plus
