Location : Oceanport, NJ

Salary Range : $85,000 - $95,000

Schedule : Full-time | Home visits required | Occasional on-call

RCM Health Care Services is recruiting on behalf of our client, a well-respected PACE (Program of All-Inclusive Care for the Elderly) organization in Oceanport, NJ , for an experienced Registered Nurse Care Manager .

This position offers an excellent opportunity for an RN who is passionate about providing comprehensive, person-centered care for older adults in both home and clinical settings. The RN Care Manager plays a critical role in assessing participant needs, developing individualized care plans, and collaborating closely with the interdisciplinary team (IDT) to ensure quality outcomes and continuity of care.

Key Responsibilities

• Provide nursing care in the center, clinic, contracted facilities, and participants' homes in accordance with each participant's plan of care.
• Conduct thorough nursing assessments and document findings in compliance with all regulatory and organizational standards.
• Participate in the 24 / 7 on-call rotation for participant triage and follow-up.
• Coordinate home care services and monitor participants' acute and chronic care needs to ensure appropriate interventions and continuity of care.
• Educate participants, caregivers, and families on self-care, medication management, infection control, and healthy living practices.
• Collaborate with the IDT to develop, implement, and revise individualized care plans.
• Delegate and oversee tasks performed by clinic, center, and homecare aides.
• Participate in quality improvement projects, utilization review meetings, and interdisciplinary case conferences.
• Maintain accurate and timely medical documentation.
