[job_card.job_description]Home health setting serving geriatric and middle-aged patients with no pediatric cases Daily caseload of 5-6 patient units Patient diagnoses include orthopedic, cardiac, neurological, pulmonary, diabetes, and other conditions Work schedule is Monday to Friday, 8 : 00am to 4 : 00pm, with coverage approximately every 6th weekend and 2 holidays No on-call or floating to other facilities required Epic EMR is used for charting and Epic experience is required Responsibilities focus on providing quality patient-centered care and developing patient care goals No pediatric patients are served in this position Participation in a multidisciplinary team including other therapists and clinicians Orientation provided with a preceptor Scrubs are provided as the required attire Parking is available at the office No more than 7 days of requested time off permitted per 13-week contract, with no time off allowed during the first week of orientation First-time travelers may be considered after interview Permanent employees must wait 1 year before returning as a traveler Active state Physical Therapist license required at time of consideration (license required if applicable) Minimum of 1 year of recent physical therapy experience in a relevant setting Current BLS certification required Experience using Epic EMR required Strong case management, time management, and organizational skills Ability to supervise PTAs and work independently References from supervisors within the past 2 years preferred Eligible candidates must reside outside a 75-mile radius from the facility No pending licenses accepted in Illinois; Missouri allows submission with pending license if requirements are met High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
