Job Description
We’re looking for a strong, confident RN Care Manager who thrives in a fast-moving telehealth environment. This role is fully remote and serves as the primary clinical contact for incoming patients, ensuring timely care coordination, patient education, and escalation of condition changes to treating providers. If you’re sharp, proactive, and able to manage patient needs with both clinical judgment and heart, you’ll fit right in.
Responsibilities
- Manage a caseload of patients and act as the escalation point for changes in condition or higher-level care needs.
- Support non-licensed care staff with ongoing care management tasks.
- Apply strong knowledge of condition management, assessments, and early warning signs.
- Perform outreach based on remote monitoring alerts that require timely action.
- Review physiological data and intervene proactively when needed.
- Present patient concerns to MDs, NPs, and PAs in a clear, structured clinical format.
- Follow established protocols and document deviations appropriately.
- Evaluate patient concerns and escalate to the correct resource, such as device support, clinical leads, or care coordination teams.
- Provide coverage for other programs when urgent staffing needs arise.
- Perform additional duties as assigned by leadership.
Required Experience
Minimum of 2 years of clinical experience in an acute care setting.Graduate of an accredited RN program.Ability to obtain and maintain multi-state licensure.At least 6 months of experience in telephonic triage and patient education.Skills and Competencies
Strong critical thinking and clinical decision-making.Excellent verbal and written communication.Consistent, accurate clinical documentation.Ability to follow protocols and apply decision support tools.High proficiency with telephonic assessment and remote triage.Able to collaborate effectively with interdisciplinary teams.Skilled in developing patient-centered care plans tailored to literacy levels and social determinants.Comfortable working with standing orders and escalating when appropriate.Ability to troubleshoot telehealth and remote monitoring technology.Preferred Qualifications
Active multi-state RN license.Licensure in Texas, New York, Hawaii, or Connecticut.Bilingual English and Spanish.Over 2 years of cardiac care experience.Ability to obtain additional state endorsements as needed.
