Home Health Care RN Case Manager

As a Registered Nurse Case Manager with Misbah Home Health Care, you will play an essential role in providing compassionate, high-quality home health care to our patients. You’ll be responsible for assessing patient needs, developing and coordinating individualized care plans, and ensuring that care is delivered effectively and efficiently.

Key Responsibilities

• Conduct comprehensive patient assessments and establish individualized care plans.
• Provide skilled nursing care in accordance with physician orders and agency policies.
• Educate patients and family members on health management, medications, and disease processes.
• Coordinate care and communicate effectively with physicians, therapists, and other team members.
• Ensure timely documentation and maintain compliance with all regulatory and agency standards.
• Manage a balanced caseload, ensuring quality care and efficient scheduling.

Qualifications

• Current Registered Nurse (RN) license in the State of Indiana.
• Minimum of one (1) year of recent clinical experience in home health.
• Strong critical thinking, organizational, and communication skills.
• Proficient in electronic medical records (EMR) documentation.
• Reliable transportation and a valid driver’s license.
• Strong knowledge of OASIS documentation and Medicare home health regulations (preferred).
