As a Clinical Liaison, you will be at the forefront of hospice care, working to nurture and grow relationships with hospitals, physicians, and other key referral sources. Your primary responsibilities will include:

• Coordinating and generating hospice referrals through existing and new accounts.
• Identifying and developing new market opportunities.
• Educating healthcare professionals and community members on hospice services.
• Ensuring a smooth transition of care for referred patients.

This role requires excellent communication and collaboration skills, as you will interact daily with patients, families, medical professionals, and community partners. Your efforts will be vital in ensuring continuity of care and accurate documentation.

Qualifications:

• A genuine desire to serve patients and families with compassion.
• Education: Graduate of an accredited school of nursing.
• Licensure: Active Registered Nurse or Respiratory Therapist license required. Must meet credentialing standards of the healthcare facility.
• Experience: Minimum of 2 years in a clinical care setting or hospice preferred.
• Requirements: Reliable transportation and capability to sit, stand, bend, lift at least 25lbs, and accommodate the weight of an average adult.
