Description :

This position works directly with patients and their families to provide support.

Guides families with financial and housing needs to available resources.

Works with state and federal agencies to provide information for placement needs.

Identifies custodial issues and refers patients in crisis to behavioral health services.

Handles referrals and consults from primary care providers within the scope of responsibilities.

Provides encouragement, coaching, and linkage with community services that support each patient's unique healthcare needs.

Key Essential Functions and Accountabilities :

Interviews patients or their designee to obtain relevant psycho-social information.

Determines the effect of illness upon the patient and the patient's family.

Participates in the development of an interdisciplinary plan of care.

Coordinates the discharge planning process and makes referrals for appropriate services.

Provides therapeutic support and education to patients and families.

Assists families in understanding medical recommendations.

Makes interventions and professional support referrals as needed.

Participates in interdisciplinary team meetings and patient care conferences as required.

Manages assigned caseload and documents results of assessments in the patient's medical record.

Ensures documentation is timely and meets regulatory compliance standards.

Performs other duties as necessary to support organizational vision, mission, and values.

Education, Certifications, and Licenses Required :

Must be a Licensed Clinical Social Worker (LCSW) or a Licensed Master Social Worker (LMSW).

Experience Required :

Minimum of 2-4 years in an inpatient or outpatient healthcare environment in social work.

Must be at least 21 years of age.

Knowledge and Skills :

Understands sound ethical practice in conjunction with established ethics.

Knowledge of basic nutrition and disease prevention recommendations.

Familiar with case management principles and applications.

Demonstrates customer service and recovery skills.

Awareness of community support resources.

Skills :

Effective communication to report patient activities and deviations from normal.

Ability to interact positively with patients and families.

Competence in using computers and electronic health record systems.

Ability to communicate effectively in oral and written forms.

Empathetic and supportive listening skills.

Ability to develop interpersonal relationships to work effectively as a team member.
