Resident Care Coordinator

We are seeking a leader!  Hours :   6 : 00 am - 2 : 30 pm

• Support HWD by managing Assisted Living team members.
• Schedule Assisted Living team members.
• Participate in Care Plan meetings with families and residents and manage ISPs as resident needs change.
• Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.
• Participate in weekly at risk meetings with HWD ED MCC / MCD to ensure leadership team is aware of all resident needs.
• Review and implement all necessary processes for optimal operational efficiency such as- shower schedules laundry schedules caregiver assignment sheets etc.

Qualifications :

Level of Formal Education : HS or GED

Area of Study : General

Years of Experience : 3-5 years caregiver experience preferred

Type of Experience : CAN experience preferred scheduling communicating with families

Special Certifications : Prefer CNA

Language Skills : Acceptable fluency in English in order to perform job duties and speak read write and communicate with supervisors coworkers residents and applicable third parties.

Technical Competencies : Basic computer skills Excel for scheduling

Skills and Ability :

Personal Attributes : Calm personality that possess the ability handle stressful situations with an easy demeanor ability to multi task and follow through and self-motivated.
