Schedule :

This role will be 32 hours each week. The particular rotation can be discussed at time of interview.

What You’ll Be Doing :

As a Licensed Clinical Social Worker (LCSW) working in Palliative Care, you'll play a key role in supporting patients facing advanced illness. You'll work with a multidisciplinary team to provide psychosocial assessments, counseling, and care planning for patients and their families.

Comprehensive Support – Provide assessments, counseling, and care coordination for adult patients with serious illnesses.

Family & Caregiver Guidance – Help patients and families navigate complex medical decisions, anticipatory grief, and difficult family dynamics.

Program Development – Contribute to new initiatives

Interdisciplinary Collaboration – Work closely with physicians, nurse practitioners, and other care providers to develop holistic care plans.
