Hanford Post Acute - Hanford, CA | Starting at $60 / hr BOA

Are you passionate about helping patients regain their ability to communicate and swallow safely? Join our dedicated team at Hanford Post Acute , where we provide high-quality, person-centered care in a skilled nursing setting.

We are currently seeking a Speech Language Pathologist (SLP) to evaluate, plan, and implement therapy services to meet the communication and swallowing needs of our residents.

Responsibilities Include :

• Evaluate patients within 24 hours or 1 business day of physician referral (with documentation if delayed)
• Develop and implement personalized treatment plans approved by referring physician
• Provide direct treatment and document patient progress in compliance with state and facility standards
• Supervise SLP Assistants and Clinical Fellows as needed
• Participate in interdisciplinary meetings, care conferences, and in-service training
• Provide caregiver education and discharge planning
• Maintain accurate and timely documentation in systems like Casamba and PointClickCare
• Comply with all applicable laws, regulations, and facility policies
