About The Position

URGENT HIRING FOR REGISTERED NURSE CASE MANAGER

Job Locations : Santa Rosa, CA

Job Duration : 13 weeks

GTDW : 40hrs

Shift : Day 5x8-Hour (08 : 00 - 16 : 30)

Responsibilities :

• Coordinate and manage patient care plans across the continuum of care.
• Conduct initial and ongoing assessments to determine patient needs and appropriate services.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure quality and timely care.
• Monitor patient progress, evaluate outcomes, and adjust care plans as needed.
• Provide education to patients and families regarding diagnoses, treatment options, and community resources.
• Ensure safe transitions of care, including discharge planning and follow-up coordination.
• Advocate for patient needs while ensuring cost-effective care delivery.
• Document all case management activities accurately and in compliance with regulatory and organizational standards.
• Review clinical documentation to determine medical necessity for services or level of care.
• Identify barriers to care and work with stakeholders to resolve them.
• Maintain up-to-date knowledge of insurance requirements, utilization management, and care coordination best practices.

Certificates : RN Lic and BLS

Experience : 2 year(s)

Note : 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment.

Benefits

Pay : $50-$55 / hr

WEEKLY TRAVEL PACKAGE :

Weekly Tax Free Stipend to be paid : $1,617.00

Weekly Straight Pay (Taxable) : $1,160.00

Weekly Gross : $2,777.00
