Key Responsibilities
Conduct comprehensive evaluations of members' needs and develop tailored care plans
Coordinate all aspects of member care and ensure consistent support for health improvement
Advocate for members' access to care and maintain accurate documentation of assessments and progress
Required Qualifications
3-5 years of experience in case management, social services, or healthcare
Familiarity with Medi-Cal, CalAIM, and Community Supports programs
Knowledge of healthcare systems and local community resources
Proficiency in using case management software and EHR systems
Strong interpersonal skills and ability to work collaboratively in a team environment
