[job_card.job_description]Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies. Responsible for supervising and evaluating Home Health Aides and Licensed Practical / Vocational Nurses. Conduct home visits and attend periodic office meetings. Assignments may vary with a 75-mile radius travel commitment. Minimum of 5-8 patients per day, meeting productivity expectations of 30 points per week. Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits. Participate in weekend rotations and on-call duties as assigned. Complete all patient documentation within 24 hours of appointments. Be open to floating to nearby clinics if patient load is insufficient. Understand that declining patient assignments can lead to contract cancellation.
